Privacy Policy

Therapists at CHaC will communicate only to those who you authorize the disclosure of information.  If you prefer and authorize for communication with other doctors or service providers, however, CHaC therapists are happy to communicate with primary care physicians, medical specialists, nurses, dietitians, certified diabetes educators, physical and occupational therapists, or clergy.

To authorize such communication, simply provide your therapist with the contact information, and sign a “release of information’ form.
